POSITION SUMMARY
Adoray Home Health and Hospice is recruiting a full-time social worker, MSW preferred, to provide support for our home health and palliative patients and provide in-patient social work support at our partner hospital WW Health.
The hours are Monday-Friday 8:00 -4:30 pm. The morning will be spent working at WW Health providing support for their inpatients and the afternoon supporting home health and palliative patients for Adoray.
Minimum Qualifications
- Masters of Social Work degree from an institution accredited by the Council on Social Work Education.
- One year of social work experience in a health care setting required, two years preferred.
- Hospice and/or home health experience preferred.
ESSENTIAL DUTIES AND RESPONSIBILITIES
WW Health Duties:
- Provides a wide range of social work modalities with individuals, families and groups based upon a social work assessment;
- Conducts on-going assessments, including gathering comprehensive information, to use in developing psychosocial assessments, interventions, and treatment strategies;
- Implements interventions that promote patient well-being and ensures a continuum of care;
- Advocates for the needs and interests of the patient to improve delivery of services;
- Demonstrates sensitivity to and awareness of cross-cultural issues and integrates this knowledge into practice;
- Maintains documentation of social work services that reflect the patient’s pertinent information for assessment and treatment, social work involvement and outcomes, and in accordance with the treatment plan;
- Provides resources and guidance to other health care providers in the psycho-social aspects of patient care and performs related duties;
- Collaborates with health care workers in treatment planning, care conferences and recommendations;
- Assistance with facility discharge planning and discharges including those with complicated psycho-social elements and provide follow up care where needed;
- Works with law enforcement, corporation counsel, and county departments to assist with legal aspects of patient care;
- Coordinates care through the hospital stay and post discharge;
- Assists with advance care planning, providing resources and guidance across the organization
- Assist in coordinating/collaborating for internal and external payor needs (insurance companies, managed care, state/federal programs)
- Demonstrates awareness, understanding and respect of cultural diversity and patients of all ages and socioeconomic groups, and
Adoray Duties:
- Completes psychosocial evaluation/assessment visit on each assigned home health or palliative patient as assigned.
- Assesses the concerns of home health, palliative or hospice patient/caregiver on an ongoing basis.
- Assists the patient/caregiver in identifying goals and establishing priorities on an ongoing basis.
- Assists the patient/caregiver in assessing financial needs and determines eligibility and availability of appropriate community resources.
- Ensures pertinent patient information is communicated to the home health, palliative or hospice case manager and/or other members of the care team as necessary.
- Assists the home health or hospice patient/caregiver in planning for transfer from one setting to another (i.e., home to nursing home) and facilitates implementation as requested.
- Assists in planning transition of patient and caregiver if discharge or revocation from the hospice program occurs.
- Assists in facilitating effective adaptation to the crisis of illness and impending death.
- Communicates with physician and other team members as needed regarding changes in patient/caregiver status.
- Makes referrals to other appropriate health care workers and/or agencies as necessary.
- Advises other team members regarding psychosocial needs for specific patient/caregiver situations as appropriate.
- Facilitates patient/caregiver conferences to assist them in making decisions and effectively communicate.
- Participates in palliative IDT meetings a minimum of every 30 days. Contributes to care plan revisions of all patients reviewed based on knowledge and experience.
- Participates in hospice, home health care conferences or palliative care team meetings as requested.
- Rounding with partner hospital to identify potential home health, palliative or hospice patients.
- Other duties as assigned
