- Maintains resident medical records on the nursing units including:
- Setting up new chart for admissions; new forms for readmissions.

Tearing down chart for discharges.

Filing reports, notes and other paperwork in the appropriate medical record.

Replenishing charts with needed forms.

Auditing charts for completeness and thinning charts according to Care Plan Schedule.

General Record Maintenance - repairing pages, checking ID, sequencing, cleaning binders.
- Charts and checks documentation for completeness, including vitals signs, weights, CNA charting and others as assigned.

Routes and distributes various reports to appropriate parties (e.g. lab and x-ray reports, consultations, hospital records, incident reports, etc.)

Collects data and compiles reports for quality assurance and other studies as assigned.

Prepares paperwork for physician visits and facilitates completion of same. Following physician visits, reviews physician clipboards and charts and distributes documents to appropriate locations. Monitors for physician monthly progress notes, annual history and physicals, etc.

Retrieves supplies as requested and distributes tube feeding supplies as needed.

Takes resident photographs for medical records / door, marks dentures, eyeglasses, hearing aides with resident name.

Faxes physician orders to the pharmacy according to established time schedules.

Answers and routes incoming telephone calls for the entire facility.

Coordinates the scheduling of various clinics held on site (podiatry, eye, hearing).

Retrieves records and health information for authorized staff as requested, in accordance with facility policy.

When requested by the nursing supervisor, makes calls to nursing personnel for staff coverage.

Attends inservice programs and meetings scheduled for staff.

Maintains resident confidentiality; treats residents with kindness; dignity and respect; know and comply with all Resident's Rights rules; and promptly report all resident accidents and injuries to charge nurse.

As assigned, performs functions as outlined in the job description for Health Information Secretary - Purchasing Clerk.

Must maintain attendance according to facility policy. Must have regular and timely attendance.

All other duties as assigned.
