Job Description

The Structured day director has the responsibility for overseeing, planning and implementing structured day services to individuals in the program.

Schedule: Monday through Friday 8am to 4:30pm at our Limeport, PA (ACTION RECOVERY).

Assessment: Demonstrates knowledge of normal growth and development processes.

Performance Expectations:

  • Assists the team in gathering and providing relevant data to the Multidisciplinary Treatment Team.
  • Works with the Multidisciplinary Treatment Team to review the psychosocial history and assessment.
  • Consistently observes and assesses consumers for mood and behavior changes.
  • Identifies risk factors regarding health and lifestyle and reports information to the multidisciplinary Treatment Team
  • Identifies emotional barriers as it relates to the consumer’s diagnoses and reports information to the multidisciplinary Treatment Team
  • Assesses consumer’s involvement in work, family, and community and adjustment to changes and limitation associated with aging and medical and mental health diagnosis and reports information to the multidisciplinary Treatment Team.

Planning: Demonstrates the ability to assist the team in carrying out individualized, consumer-centered treatment plans that structure the course of treatment.

Performance Expectations:

  • Within the context of the multidisciplinary Treatment Team Meeting assists the team identify goals, measurable objectives, outcomes, and interventions to assist consumers in attaining goals.
  • Plans activity that best assists the consumer in attaining goals on the treatment plan.

Implementation: Demonstrates the ability to assist the team in moving consumers toward problem-resolution within an acceptable time frame.

Structured Day Management

  • Complete paperwork upon admission to the Action Recovery unit.
  • Complete leisure surveys with consumers
  • Re-evaluate program member’s with structure day assessment annually.
  • Plan and facilitate therapeutic and recreational groups that foster personal growth.
  • Organizes program member interactions in the community.
  • Provide a safe and therapeutic environment.
  • Maintain and follow a budget set forth by administration for supplies.
  • Provide brain injury training and in-service to new employees to the Action Recovery Unit.
  • Educate structure day staff regarding compensatory strategies and techniques unique to each program member.
  • Facilitate monthly house meetings.
  • Supervise staff with structure day duties.
  • Oversee daily process of structure day.
  • Ensure completion and quality management of Residential Habilitation Notes.
  • Develop and distribute weekly client schedules.
  • Complete structure day billing.
  • Complete billing and monthly documentation (i.e. clinical summaries).
  • Provide brain injury training and in-service to new employees to the Action Recovery Unit.

Case Management

  • Complete initial and ongoing RASP evaluations for all consumers as needed.
  • Provide advocacy for clients, ensuring that their needs are being met.
  • Act as the primary liaison between clients and community resources, including federal, state, and county agencies. Ensure that all entitlements are received. Work with insurance carriers to ensure coverage.
  • Maintain ongoing contact with families or designated person(s) of clients
  • Maintain positive, supportive relationships with clients, spending individual time on bi-monthly basis or as requested by the client.
  • Arrange for clients to attend appropriate and therapeutic psychiatric programs.
  • Assist clients to set and achieve educational or vocational goals.
  • Encourage clients to maintain their ADL’s, hygiene, diet, grooming, etc. Coordinate care with Brain Injury Specialist if client requires assistance.
  • Contact psychiatrist if the client has any medication complaints, side effects, or problems. Arrange for appointments as necessary, accompany client if need be.
  • Work with Medical office staff to relay information to physicians regarding client medication compliant, side effects, or health issues.
  • Assist in developing appropriate discharge plan.
  • Establish and maintain accurate and comprehensive social service files, including: admission paperwork, annual and semi-annual assessments, progress notes, discharge summaries, and inspection review.
  • Accompany clients or arrange for Brain Injury Specialist to accompany clients to appointments as needed or as appropriate.
  • Handle crisis with clients and assist other staff in resolving problems/crisis situations as they arise.

Evaluation: Demonstrates the ability to continuously evaluate the consumers’ response to treatment approaches and reports the need to modify the treatment plan to achieve the goals.

Performance Expectations:

  • Interprets behavior in activities group in relation to consumer’s age and uses the information to assist the team in updating a treatment plan.
  • Participates in interagency communication as it applies to facilitating consumer’s treatment plan.
  • Assists in the periodic review of the consumer’s treatment plans.
  • Consistently and thoroughly records consumer’s progress.
  • Monitor required trainings of structure day staff.

Safety: Demonstrates ability to set appropriate limits.

Demonstrates skills in maintaining consumer safety in the treatment setting.

Performance Expectations:

  • Monitors interactions and appropriately intervenes when boundaries are violated.
  • Monitors and maintains the environment for safety of the consumer and staff.
  • Recognizes potential and/or emergency situations and intervenes appropriately.

OTHER ESSENTIAL PERFORMANCE EXPECTATIONS

  • Abides by the employee health policies.
  • Attends mandatory training sessions at orientation and annually there after: General Safety (fire and disaster practices); Confidentiality, Abuse and Neglect, Infection Control. In addition, attends an extra 10 hours of training annually.

REPORTING REQUIREMENTS

  • Reports to the multidisciplinary Treatment Team on the progress of consumer’s treatment goals and other relevant issues such as, attendance, and participation with the group sessions.
  • Reports serious incidents and/or breaches in policy and procedure to the Recovery Center and Personal Care Home administrators.

QUALIFICATIONS

1. Education:

1. Education:
Required: Bachelor’s Degree, Master’s Degree preferred in Counseling, Social Work, or related field

2. Experience:
Required: One (1) year paid experience in the human services field or supervised practicum as part of the degree program. One (1) year experience in the Brain Injury field and Five (5) years’ experience in running programs with individuals with disabilities.
