This is an ideal role for a professional with proven ability to provide effective supervision grounded in knowledge and experience in the field of aging, or other relevant work experience. Working in close collaboration with a multidisciplinary team of professionals, under the supervision of the Home Care Supervisor. A successful candidate is passionate about supporting caregivers working with older adults and adults with disabilities.

REQUIREMENTS
-
High school diploma required.
-
A background in geriatrics, home care or related healthcare industry is highly desirable
-
At least 3 years of successful supervisory experience

Excellent listening and communication skills, both written and verbal

Demonstrates reliability and efficiency by timely follow- through on assigned tasks.

Maintains high level attention to detail: Detail oriented in attending to the day to day needs and concerns of clients and staff requiring follow up while not allowing important tasks to fall through the cracks.

Able to establish positive rapport with supervisees, inspiring mutual trust, and high quality performance

Proficiency with computer software programs: Google drive, documents & spreadsheets, time management software, email, calendar and mobile applications

Must have a valid California driver’s license, insurance, and reliable transportation
RESPONSIBILITIES
-
Training and oversight of caregivers assigned to your caseload, including screening new candidates, onboarding new hires, ongoing mentoring, coaching and providing effective feedback to improve performance.

Ensuring that care providers adhere to Sage’s Quality of Life (QOL) approach to care and other high standards of service.

Developing creative solutions with your team to overcome challenges and to keep clients engaged.

Providing quality assurance including supervisory visits to ensure team members deliver care that meets our high quality standards.

Managing team schedules, including on-call coverage and utilizing daily shift reporting tools to evaluate care provided.

Collaborating with Care Management, Hummingbird and other teams at Sage to ensure seamless coordination of care for the client.

Participates in on-call rotation (approximately once every 7 weeks)
