Job description: Senior Coordinator of Supportive Services
Reports: CEO/Executive Director
Status: Full-Time/Exempt
Organization Background and Current Status: The Renaissance Collaborative, Inc (TRC) is award-winning non-for-profit housing and supportive service agency serving the historical Bronzeville Community on Chicago's mid-south side. The Renaissance Corporation is a non-for-profit organization whose mission is to promote a holistic agenda for the homeless, low- and moderate-income residents of the near and mid-south Chicago community to enable them to rebuild their lives by providing comprehensive services and support. TRC is dedicated to providing innovative affordable housing environments that renew people and build healthier communities. The Renaissance Collaborative, Inc is a thirty-year-old non-profit organization. TRC combines housing and on-site supportive services for the residents it serves. It works in partnership with other community civic, economic, and social service organizations to achieve its mission. The goals of TRC are to:

- Design and deliver programs of wellness that address the social, physical and mental well-being of participants and residents.
Position Description:
- This manager facilitates, coordinates, and assists in The Renaissance Corporation's efforts to rebuild lives of homeless low- and moderate-income individuals, families, and support the organization's mission, goals by helping them become economically self-sufficient. The position leads the area of policy, reporting requirement and review annual physical exams of residents that enable and promotes accountability and self-sufficiency. This position: 1) manages all staff in residential operations and 2) reviews intake assessment processes and procedures to ensure they conform to the rules and policies. This position helps monitor and evaluate client progress and when required assist in making appropriate client referrals. This manager is a vital component to the Residential Service Team and oversees all residential services for both Renaissance Apartments, TRC Senior Village and soon the Intergeneration Housing initiative.
Reports to: Executive Director/CEO
Supervises: Health & Wellness Manager, Life Skills Manager, Education & Employment Manager: Property Managers of both Renaissance Apartments and Senior Village, Employment Placement Manager, and Coordinator of Service at Senior Village.
Essential Duties and Responsibilities:
Supervision and Program Quality Monitoring (approximately 50% of the position)
- Provide supervision to staff in both facilities and services ensuring accountability and compliance are met for the agency.
- Provide supervision to staff who provide social services to elderly residents of TRC Senior Village.
- Supervise general as well as formal case management what includes formulation and implementation of realistic treatment plans that resolve problems and promote maximum level of client self-sufficiency which will enable the residents to continue to live independently.
- Demonstrates skills in securing, interpreting and evaluating data, recognizes and understands aspects of both normal and pathological aging, reveals sensitivity to client needs, strengths, and weaknesses, forms and follows through on realistic plan care.
Administrative Documentation and Reporting (30%)
- Provide policy monitoring to ensure the agency meets reporting requirements for both facilities, services and make recommendations on policies and procedures to Senior Management.
- Document and resident encounters and progress within 48 hours of contact in file including contract agreements for unpaid rent.
- Maintain up to date activity records and complete end of the month paperwork as indicated in operation manual (this includes HMIS, IDHS, SPH, and HUD.
Special Client Relationship (approximately 10% of the position)
- Provides referrals to the I & R Unit and Department of Aging’s 800 Unit Elder Abuse Hot Line, conduct extensive in-home assessments; determine the services each client needs, make referrals for chore/housekeeping, transportation, senior companion, friendly visiting, advocacy, and home-delivered meals, makes appropriate referrals outside the agency for client services; knows a wide range of community resources; advocates for clients and work with others in the community to bring comprehensive services to the residents. Work with family and informal caregivers to maximize assistance for clients; counsels clients and their family members through a wide variety of issues/problems such as depression, abuse, or grief, offers some services to home-bound clients unable to attend conventional mental health agencies; explains to clients their opportunity to contribute to the cost of their auxiliary service-chore; home delivered meals or transportation. Encourages residents to participate in building activities. Utilization and control of physical and financial resources: responsible for using office supplies and equipment appropriately; for operation person automobile for work in a safe manner and without excessive mileage; maintains program budget and ensure that he service expenses never exceed the total funds allotted to the operational budget. When necessary, shifts funds from one budget line item to another with approval from Executive Director.
- Contracts & Public Relations: Responsible for contacting and meeting with medical providers, community organizations, public and private agencies with Executive Director/designee as needed in development of client care plans, activities, and education. Involves community groups and organizations in the needs and activities of the residents
Resident and Community Development (10%)
- Assist in developing and coordinating social, cultural and education activities with residents.
- Oversees the documentation by staff on any resident encounter and progress within 48 hours of contact in file so other staff will know, including contract agreements for unpaid rent.
Job Requirements
- Master’s degree in social work (Required)
- Experience working with, counseling of adults homeless/ex-offenders, mental health, substance abusers, and/or veterans/senior populations
- Must have the ability to act in the capacity of a coach, mentor, facilitator, advisor, or a motivator of a team of professionals.
- Must be organized and manage time in an efficient manner.
- CADC certification (Required) LSW a plus
- Team Management: 1 year (Required)
- Driver's License (Preferred)
