Job Description
The Pharmacy Outreach Program seeks a temporary part-time 12-hour a week Pharmacist to provide direct services to a primarily older adult population. Services include providing medication screenings and counseling, assisting with financial barriers, and delivering health education programs in the community.
- Assess cases to identify potential problems or adherence concerns; develop plan in conjunction with client to reduce the risk of problems and to increase adherence
- Research and provide medication therapy information and education to clients and/or health providers
- Research and interpret federal, state, and local benefit programs, assistance options, and other cost-saving strategies to determine best financial solution for clients
- Communicate effectively to convey plans, medication therapy information, and other findings with medical professionals and the public
- Perform follow-up to assess clinical outcomes including appropriate use of medications, compliance, and surveillance of adverse drug events
- Create and deliver community-based health education programs at community-based sites in Massachusetts and via Zoom as needed
- Provide Medicare Part D evaluations to ensure medication coverage at an affordable cost
- Oversee APPE students and provide experiential content as needed
- Create and maintain confidential client profiles as required by HIPAA
- Assist Director in creating programmatic, outcome data, and cost analysis reports
- Additional responsibilities may be assigned by the supervisor.
- Contributes to sustaining and enhancing an inclusive working and learning environment.
Required:
- PharmD
- Currently licensed and registered as a pharmacist in the Commonwealth of Massachusetts
- Strong knowledge of MS Office, electronic health records
- Experience creating and delivering community and zoom-based health education programs
- Experience counseling the public regarding medication therapy and cost-solutions
- Excellent verbal and written communication skills, including ability to effectively communicate
- A satisfactory result of a CORI background check is required
- Demonstrated commitment to valuing diversity and contributing to an inclusive working and learning environment
Physical Requirements:
- Standing, sitting, manual dexterity, ability to move between offices and floors of buildings
- Ability to meet deadlines and perform well under pressure
- Ability to drive to community-based sites
- Must be able to talk, listen and speak clearly on telephone
