The Visiting Nurse Association is seeking Companions to perform home management services within the client’s home. Provide companionship to those individuals requiring socialization and minimal guidance to assure a protected environment. Integrate and demonstrate the Core Values of the VNA in all aspects of work responsibilities. DFWP EEOC
1. Work under the direction/supervision of the Registered Nurse (RN) Case Manager
2. Provide companionship to the client by performing duties such as: reading, talking, listening, related communication and
psychosocial activities
3. Provide light housekeeping tasks such as meal preparation, dishwashing, laundering client’s personal garments, linen change,
dusting and vacuuming client’s room. Provide pet care if needed.
4. Provide transportation to physician appointments, errands, community activities, etc
5. Remind client to take self-administered medication
6. Assist in the maintenance of a safe and healthy environment and use/store equipment and supplies in a safe, appropriate
manner
7. Maintain a record of services by writing routine reports and correspondence.
8. Inform Scheduler of changes in assignment
9. Observe and report physical and mental changes to RN Case Manager, Patient Care Manager or Director
10. Report accidents or unusual incidents immediately to the RN Case Manager, Patient Care Manager or Director
11. Follow HIPAA guidelines and maintain all protected health information (PHI) obtained in the course of work as confidential and,
as such, should not be a matter of discussion with co-workers or others outside of the Agency
12. Comply with VNA policies and procedures; Follow VNA safety policies/procedures. Report accidents or unusual incidents
immediately to the supervisor and People and Culture
13. Attend required meetings and complete mandatory education by the department deadline
